Febrile illness of two weeks or shorter is considered as acute febrile illness. Acute febrile illness is also referred as acute undifferentiated fever because of its short duration and absence of clinical features pointing to organ specific illness. There is no international consensus definition however, fever patients with localizing symptoms and signs are excluded e.g., acute pneumonia, acute meningitis, acute urinary tract infection etc.References

National Academy of Science (US) published in 2005 that one third of spending in health care is wasteful. Miyakis et al reported that 27% of investigations done at the time of admission and 63% on subsequent days are avoidable. The American Board of Internal Medicine (ABIM) under auspices of ABIM Foundation launched in April 2012 with nine speciality societies an initiative with a goal of promoting discussions between physicians, patients and other public groups that would lead to avoidance of unnecessary tests, procedures and treatments and thus reduce waste in health care system. This initiative is called Choosing Wisely R campaign.- Blood Transfusion : Liberal or Restrictive ?

Transfusion of blood and blood components is used in therapy for many decades. It is frequently done on the assumption of the treating doctor that it will be beneficial to the patient. On many occasions this assumption may not be true and not supported by well designed randomised controlled trials. Deep vein thrombosis (DVT) and pulmonary embolism (PE) are potentially serious clinical entities which can result in fatal outcome. Results of studies published have shown that DVT and PE (together referred as venous thromboembolism) in Indian patients are as much common as occurring amongst Western countries. Globally, incidence of VTE is 0.75-2.69 per 1000 population and in population 70 years or more incidence is 2-7 per 1000 population. However, the VTE risk perception amongst treating Indian physicians is variable and results in poor implementation of prophylaxis for thrombosis as compared to other countries. It has been reported that 53.6% of hospitalised patients (surgical 63.3%, medical 44.7%) were at risk of for VTE and more than 80% of these patients did not receive prophylaxis for VTE as recommended by American College of Physicians.References
